Latest updates in the Open Protocol Combined Template

The latest changes fix the issue of ID cells (relevant for XML, not for managers). The other fixes are as follows:

Fixed an error in the ROL banding table
- changed 15%-16.5% to 15%-17.5%
Fixed tab 17 name to make it consistent with other tabs
Fixed tab 18 to request USD rather than USDm
